How to create a Questionnaire plugin in Jira

My idea is that the project lead can set up a questionnaire for project kick-off prerequisites.
All the project members need to complete the questionnaire and the questionnaire answers can be modified again by the member.
Once the member submits the questionnaire, the project lead will receive the result by email or check the result out somewhere in the project.
Then the project lead can assign specific tasks to the member according to her/his questionnaire result.

The questionnaire can be placed on Confluence or it can pop up when the member reaches the project page if the answers are not all “Yes”.

I think this may be big engineering, I hope it can be implemented quickly.
Do you have any suggestions about that?
