Web Hook and JIRA Event Triggers Stopped Firing

I am trying to invoke a web trigger on my Forge app using JIRA Project Automation, but it is returning a 424 Error publishing web request "Failed Dependency". This was working and triggering my function in Forge, but started returning this error consistently. The issue event triggers have also stopped working.

Any ideas how to remediate?

We are getting 100s of invocation errors per hour that are unhandled exceptions. Any ideas how to fix?
