Ticket time tracking and auto questionnaire after ticket completed

Hey folks, I’m hoping someone here can tell me how I can track and display the time between a ticket being submitted and an assignee being appointed? Is there a dashboard gadget anyone can recommend?

I also, I’d like to send a 5 question questionnaire to a ticket reporter once a ticket is complete. Can anyone advise me on how I could go about doing that?
